Registration Packet for Schools

Find the school below and click on file description to download and save the file to your computer. 

You will need Microsoft Word  or compatible program to open the files. You can use Word to edit names on the Art or performance certificate.  If you are having problems opening the file, you may use the Word Viewer 2007 from Microsoft (free download here) to view and print the files. If you still have problems email Beth Peter at beth@famearts.org to obtain a pdf copy of the file for your school. **Note in the art flyer, the parking garage fee should be $2.00!**
